Apply to receive services from The metta birth project
The first step in receiving all the amazing services Metta Birth has to offer is filling out the online application at the bottom of this page. If you have access to a printer, you can print the complete application document and fill it out prior to your initial meeting. If you do not have access to a printer, please plan on arriving 20 minutes early for your appointment to fill it out.
We will look over your application and respond within 2 business days to set up a time to go over your application with you in person.
You will meet with one of our intake coordinators to discuss your needs and wishes, estimated due date, financial ability and more. Refer to the list of needed items below the online application. Bring copies or originals we can copy to your initial meeting.
We encourage you to write down any questions you might have for us and do not be afraid to ask. We want you to feel comfortable with us as we will be getting to know each other over the course of your pregnancy. It is never too late (or too early) to secure services with us. Birth doula availability is based on a first come, first serve basis. We offer a rotating schedule of classes, including 'crash courses' as needed, and will try to fit you on our calendar if at all possible.
Your information is treated with privacy and only members of Metta Birth Project will see any information you give us. As a rule, we adhere to HIPAA Privacy Rules.

Items to bring to your initial interview:
This list will also appear on the complete document (that you can print) above. All financial information should apply only to the family applying. For example, if you live with a parent or roommate, their financial information is not needed. If you share the monthly housing cost, we will need to know what you are responsible for. We do not need to know about child support orders.
If you are not responsible for housing costs or utilities, you will not be required to bring those items. A proof of address and information regarding who is responsible for those costs will be required.
- Proof of monthly housing cost (lease or mortgage statement)
- Proof of address (your utility bill will work)
- Utility bills for gas and electric
- Three months worth of paycheck stubs
- Proof of pregnancy (a letter provided by a medical professional, ultrasounds will not be accepted as proof)
- State-issued ID or driver's license or birth certificate (if applying as a couple, we will need ID for both parents)
- Proof that you are receiving prenatal care under a provider
Please note: Mothers who will be under 20 at their estimated due date will need only to provide proof of pregnancy and identification/proof of age..
